How to Appeal

*You have 20 business days to appeal

If you disagree with the ministry’s reconsideration decision:

Step 1

Fill out a Notice of Appeal (PDF) form.

Step 2

Send us your Notice of Appeal form.  You must send your Notice of Appeal form within 20 business days of receiving your ministry reconsideration decision.  If not, you will lose your right of appeal.

You can give us more information and evidence after you submit this form if you need to.
